POSITION SUMMARY

Key Medical Care, A SilverStone Health Company, is seeking a Care Coordinator, with a MA or LVN background. The Care Coordinator is responsible for performing administrative and clinical duties under the direction of a licensed healthcare provider. This role helps facilitate communication with patients and providers to provide high-quality patient care.

At SilverStone Health we are reimagining the at-home continuum of care. We guide, empower, treat, and support patients and families facing a chronic or terminal illness, enabling them to make the most informed decisions throughout their healthcare journey.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Include the following.  Others may be assigned.

  • Consistently review assigned patients to make timely and appropriate patient care calls.
  • Schedule patients for routine tele-visits with providers.
  • Communicate with providers, including any outside specialists involved in patient care.
  • Assist in acquiring patient records.
  • Creating and maintaining care plans based on provider approved care plans for patient centered care.
  • Ensures care plans are shared with patients and families based on CMS guidelines.
  • Provides patient and family education.
  • Ensures compliance with CMS guidelines for Chronic Care Management.
  • Assists in enrolling new patients to the Guided Care+ program.
  • Communicate with interdisciplinary teams, such as social workers and home health for patient needs.
  • Reports errors or issues promptly to supervisor.
  • Maintains security of medical records and patient’s health information according to HIPAA standards.
  • Communicates with providers regarding immediate concerns with patients.
  • Follows through with patient inquiries, requests and complaints. Forwards difficult and non-routine inquiries or requests to appropriate level for resolution.
  • Provides occasional work guidance, technical advice, and training staff.
  • Additional duties may be required by the care coordinator

Education / Experience:  Include minimum education, technical training, and/or experience preferred to perform the job.

Required:

  • 2+ years experience in health education, patient navigation, and/or health clinic patient assistance field
  • High School Diploma or GED
  • Valid Texas Driver’s license
  • Must have reliable transportation

Preferred:

  • Certification for Medical Assisting 
  • Active nursing license on Texas Board of Nursing in good standing 
  • Associates Degree
  • Bilingual (Vietnamese, Chinese, Arabic and/or Spanish with English)

  • While performing the duties of this job, the employee is regularly required to stand, sit, talk, hear, stoop, kneel, and use hands and fingers to operate a computer and telephone keyboard reach.
  • Frequently required to stand, walk, sit, use hands to feel, and reach with hands and arms, includes ability to walk through skilled nursing facilities. 
  • Possess the ability to fulfill any office activities normally expected in an office setting, to include, but not limited to:  remaining seated for periods of time to perform computer based work, participating in filing activity, lifting and carrying office supplies (paper reams, mail, etc.)
  • Light to moderate lifting 20-25 pounds 
  • Fine hand manipulation (keyboarding)
  • Ability to uphold the stress of assisting patients of diverse backgrounds.

  • The noise level in the work environment is usually low

  • Position will need to adhere to any immunization, vaccination, or other requirements in place at our client sites in accordance with their policies and procedures.
EXPECTED PAY:
  • 18-25 USD per hour

  • Local travel to main Silverstone Health Office in downtown Dallas.
  •  4100 Harry Hines Blvd Suite #375, Dallas, TX 75219
  • Hours: Mon-Fri 8am-5pm
