POSITION SUMMARY

Key Medical Care, A SilverStone Health Company, is seeking a Geriatric Nurse Navigator, with an active LVN license. The Geriatric Nurse Navigator will be part of a new program with the goal to support elderly patients and their families as they navigate the complexities of healthcare post-discharge from the hospital. This role involves communicating with patients, coordinating care, facilitating communication among healthcare providers, and ensuring that geriatric patients receive personalized, compassionate support throughout their healthcare journey. The navigator will play a critical role in enhancing the quality of care, improving patient outcomes, and fostering a positive patient experience. This particular role will be primarily located at Carrollton Regional Medical Center, 75010.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Patient Assessment: Ability to understand geriatric patients’ health assessments, assist in their comprehension of their health needs, and inquire about their preferences and social circumstances to assist in the coordination of their personalized care plans.
  • Education and Advocacy: Educate patients and families about available healthcare services, treatment options, and resources. Advocate for patients’ needs and preferences within the healthcare system.
  • Care Coordination: Assist with follow up care by collaborating with interdisciplinary teams, including physicians, nurses, social workers, and therapists, to coordinate care across multiple settings and specialties.
  • Appointment Management: Assist patients in scheduling appointments, coordinating transportation, and managing follow-up care to ensure timely access to services.
  • Documentation: Maintain accurate and confidential records of patient interactions, care plans, and referrals in compliance with organizational policies and regulations.
  • Quality Improvement: Participate in quality improvement initiatives and data collection efforts aimed at enhancing care delivery for geriatric patients.
  • Health Promotion: Promote preventive care and healthy aging by providing information about wellness programs, screenings, and community resources.

QUALIFICATIONS

  • Current licensure as a licensed vocational nurse (LVN) 
  • Previous experience in geriatric care, case management, or patient navigation is highly desirable.
  • Strong understanding of the unique healthcare needs of elderly patients and knowledge of relevant community resources.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work collaboratively in a team-oriented environment while also being self-directed.
  • Proficiency in electronic health record systems and healthcare technology.

Education / Experience:  Include minimum education, technical training, and/or experience preferred to perform the job.

Required:

  • Active LVN license in good standing 
  • Valid Texas Driver’s license
  • Must have reliable transportation

Preferred:

  • 2+ years experience with clinical, case management, or related duties 
  • Bilingual (Vietnamese, Chinese, Arabic and/or Spanish with English)

  • Frequently required to stand, walk, sit, use hands to feel, and reach with hands and armsIncludes ability to walk through hospital-based departments across broad campus settings, including Emergency Department environments
  • Possess the ability to fulfill any office activities normally expected in an office setting, to include, but not limited to:  remaining seated for periods of time to perform computer based work, participating in filing activity, lifting and carrying office supplies (paper reams, mail, etc.)
  • Light to moderate lifting 20-25 pounds 
  • Fine hand manipulation (keyboarding)
  • Ability to uphold the stress of assisting patients of diverse backgrounds.

EXPECTED PAY:
  • 28.50 - 30 USD per hour 

TRAVEL

  • Primary worksite at Carrollton Regional Medical Center - 4343 N Josey Ln, Carrollton, TX 75010
  • Occasional travel to Silverstone Health Office in downtown Dallas - 4100 Harry Hines Blvd Suite #375, Dallas, TX 75219
