Integrative Emergency Services, LLC (“IES”) is looking for a Case Manager who will provide case management services to patients with substance use disorder and have familiarity with populations’ cultures. Will coordinate project service and activities, including training, communication, and information dissemination for patients serviced under SAMHSA (Substance Abuse and Mental Health Services Administration) ED-ALT Grant TI-23-010 from Beta to Alpha in affiliation with our clinical partner(s). Will work with participants to help them meet their individual goals. The Case Manager will have a minimal caseload of 25 patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Include the following.  Others may be assigned.

  • Utilize advanced skills to assist in collaborating, developing, implementing, monitoring, and evaluating the case management process while utilizing EMR (Electronic Medical Record).
  • Act as a consultant to others for decision-making and problem-solving related to the patient’s disease process.
  • Assess the patient’s needs related to the medical diagnosis, treatment plan, financial resources, and psychosocial issues. Collect and analyze information in collaboration with the patient’s family, health care team, etc. Communicate these identified needs to other members of the health care team.
  • Intervene and collaborate with the healthcare team when variances occur in the multidisciplinary discharge plan. Initiate and/or participate in collaborative efforts that provide ongoing evaluation of goal attainment.
  • Identify and assess patients at high risk for repeat admissions and/or high utilization.
  • Complete Community Case Management referrals on patients that meet the stated criteria.
  • Document each component of the case management process and related activities in accordance with departmental guidelines. Meet minimal caseload expectations.
  • Apply standard utilization (InterQual and/or Milliman) criteria for clinical reviews to determine medical necessity including intensity of service, severity of illness and discharge screens.
  • Serve in consultative/advisory role to other members of the healthcare team assessing, planning, implementing, monitoring, and evaluating the case management plan.
  • Serve as a patient advocate, using problem-solving techniques and collaborative referrals to facilitate Network and community resources access.

  • Ability to work with people of all social, economic, and cultural backgrounds.
  • Ability to demonstrate flexibility, open-mindedness, and adaptability to change.
  • Ability to demonstrate patience and tact when dealing with patients, families, and other staff.
  • Ability to work independently and exercise sound judgement in interactions with physicians and patients and their families.

Education / Experience:  Include minimum education, technical training, and/or experience preferred to perform the job.

Required:

  • RN, LCSW, LMSW, or LPC license that is active and in good standing.
  • Minimum 2 years of clinical experience in an acute care setting or community-based organization.

Preferred:

  • Master’s Degree from an accredited school of professional nursing or social work.

  • Frequently required to stand, walk, sit, use hands to feel, and reach with hands and arms. Includes ability to walk through hospital-based departments across broad campus settings, including Emergency Department environments
  • Possess the ability to fulfill any office activities normally expected in an office setting, to include, but not limited to:  remaining seated for periods of time to perform computer based work, participating in filing activity, lifting and carrying office supplies (paper reams, mail, etc.)
  • Occasionally lift and/or move up to 20-25 pounds 
  • Fine hand manipulation (keyboarding)

TRAVEL

  • Work will primarily be done at or near Baylor All Saints Medical Center in Fort Worth (76104)
  • Future travel to Irving and Grapevine locations may also be required
  • Some travel including overnight stays may be required.
